When you call to make an appointment, the receptionist will ask you a few questions. Be sure you have all the following information available when you call:

  3. Phone number
  4. Date of birth
  5. Social security number
  6. Insurance

  8. Marital status
  9. Emergency/Family contact
  10. Diagnosis
  11. Primary care physician
